１. 全国調査の実施法法の概要

全国の造血細胞移植の実態を把握する調査は、小児では日本小児血液学会骨髄移植委員会が1983年から行っていたが、成人をも含めた全国調査がはじめて行われたのは1993年の日本造血細胞移植学会（1995年12月に日本骨髄移植研究会から改名）の第1回全国調査である。この第１回目の調査は成人の移植については、1990年7月から1993年6月までに実施された移植例を対象とし、小児成人共同で神奈川県立こども医療センターの事務局が実施した。1994年から1997年までは、小児での調査については神奈川県立こども医療センターの事務局で、成人での調査については愛知県がんセンター研究所内の事務局で実施された。造血細胞移植に関する情報の収集や提供についての効率化を図るために、平成10年度から全国調査の実施を両事務局が協力して実施することになり、全国調査集計データが統合され、報告書が作られた。骨髄移植推進財団の移植事業（JMDP）を介して行った非血縁者間骨髄移植の追跡調査については調査が重複するため、1999年調査から成人については全国調査の調査対象から除外され、JMDPのデータを統合した。2000年4月から健康保険が適応となった同種末梢血幹細胞移植については、同種末梢血幹細胞ドナー登録センターを介した「レシピエント短期調査票」を用いた調査が行われており、その調査票のコピーを送付していただき、入力対象とした。ただし、「レシピエント短期調査票」がなく、全国データ集計事務局に直接郵送された症例についてはこれを集計対象に含めた。 　2003年3月に愛知県がんセンター研究所内にあった事務局が名古屋大学予防医学教室に移転した。 　1991年以降の移植についての症例報告があった全国調査協力チーム数は368施設である。本年の新規登録症例は2002年1月から12月までの１年間の移植例を対象としたが、報告もれ症例については過去に溯って登録して頂いた。新たに登録に参加した施設については過去の移植例も含めて登録を要請した。

The Bone Marrow Transplantation Committee of the Japan Society for Pediatric Hematology (JSPH) launched the nationwide survey on stem cell transplants in 1983 for patients who underwent stem cell transplants in pediatrics departments. Concerning adult cases, the Japan Society for Hematopoietic Cell Transplantation (JSHCT), former the Japan Society for Bone Marrow Transplantation, conducted for the first time in 1993, as well as the child cases. The adult subjects in the first survey were patients who underwent transplants between July 1990 and June 1993. The survey was conducted by the Office of the Kanagawa Children’s Medical Center of JSPH. From 1994 to 1997, the annual surveys were performed separately; by the Office in the Kanagawa Children’s Medical Center for pediatrics departments, and by the Office in the Aichi Cancer Center Research Institute for the other departments. In order to collect and provide information on stem cell transplants in an effective way, the two Offices collaborated in 1998, which allowed the integration of data on all transplants throughout Japan. For the cases with unrelated bone marrow transplants through Japan Marrow Donor Program (JMDP), a follow-up study has been conducted by JMDP. In order to decrease institutions' burden of dual reporting, those cases have been excluded from JSHCT surveys since 1999. Instead, the data on JMDP cases were retrieved from JMDP database. In April 2000, allo-PBSCT became the subjects for health insurance coverage. The data for such patients have been collected through the Allo-PBSCT Donor Registration Center using another registration form, “Short-term Registration Form for the Recipient”. In case that the recipient’s data were sent directly to the Office of Nationwide Survey, the data were also incorporated into our dataset. 　The office moved from the Aichi Cancer Center Research Institute to Preventive Medicine in Nagoya University Graduate School of Medicine in March 2003. 　The number of teams, which reported cases of transplants between 1991 and 2002 to the surveys and/or JMDP follow-up study, was 368 in total. The subjects for the new case survey were patients who underwent a transplant between January and December 2002, although unreported transplants prior to January 2001 were also included in this survey. Additionally, the past transplants reported from new institutions were included here.
